We want your knowledge and expertise!Job Summary and QualificationsPosition summary: In conjunction and collaboration with a multidisciplinary team, this position is designed to assess and meet the ongoing psychosocial needs of the adult oncology population treated at the Paul Siebenmorgen Cancer Center at Terre Haute Regional Hospital. As well, to direct and assist in the development, implementation, and evaluation of social service programs for the oncology program. What will you do in this role:Maintains assigned patient case load in collaboration with the multidisciplinary teamReferrals are received directly from oncology care staff or as a result of the Oncology Distress Screening ToolWhen applicable and as needed, performs the Oncology Distress Screening assessment using the designated toolPerforms psychosocial assessment and evaluation with emphasis on patient and caregiver needs after a cancer diagnosis and throughout the continuum of cancer care.Provides individual and family counseling, including distress management, brief therapy, and crisis interventions. Facilitates referrals for ongoing therapy needs (to outside provider or agency) in collaboration with the team and the treating oncology provider.Works effectively and collaboratively with the multidisciplinary team to meet/manage identified needs of the patients/family members including but not limited to education, practical needs, emotional needs, spiritual care, coping skills, quality of life issues, ongoing counseling, transitions in care, and advance directives.Works closely and collaboratively with the Oncology Nurse Navigator assisting and managing ongoing assessments and referrals for identified needs including financial, food, housing, transportation, employment, language and cultural barriers.Assists with referrals to community resources and agencies for home care, hospice care, and/or medical equipment as needed.Documents patient’s progress, interactions, interventions, and services provided in a timely manner.
